Doing an Employee Write-Up is Bad but sometimes you just got to do it. This module not only helps you keep track of the write-up but also the number of Occurrences, Witnesses (if any) & Discipline Type. To Create a New Employee Write-Up, go to:

  • Menu > Employee Write-Up’s > Create a New Employee Write-Up > Fill in the info needed > Submit.

 

You can also upload images or files pertaining to this Employee Write-Up.

All kinds of Incidents happen at the work place. This module helps to Track, Respond Quickly & Accordingly. It helps to make corrections to prevent the same Incidents happening again. You can also Create an OSHA 300 Log, you can do so by scrolling to the bottom of the page & hit the blue button “Create OSHA 300 Log (Add Employee)”

To create an Incident, go to:

  • Menu > Incidents > Create New Incident > Fill out the details > Submit.

Please keep in mind that you can upload images of the incident pertaining to that particular Project.

Purpose:

Background:

While it is unfortunate there are times you need to write up an employee for disciplinary purposes, Contractor Foreman gives you the tools and organization to do it in a proper way.

Points to Remember:

Required fields are marked with an Asterisk (*.)

To save your work, click the blue “Submit” button.

To edit the record, click the green pencil icon in the upper right corner.

Deleting Items can only be done by a person with Admin Rights or if you are given access to do so.

If an Employee Write-up needs to be Deleted, you can do so by clicking on the “Action” button at the bottom of the page on the right.

Steps to Create a New Employee Write-Up:

  1. Navigate to Main Menu.
  2. Click on Employee Write-ups.
  3. Click on blue button “Create a New Employee Write-up” on the left.
  4. Enter the following information:
  • Employee: (Mandatory) Select from Drop-down.
  • Date: (Mandatory)
  • Discipline Type: Select from Drop-down.
  • Note:
  • Occurrence: Select from Drop-down.
  • Witnesses: Select from System List.
  • Incident: Select from Drop-down.
  • Add an Attachment: Upload all Photos & Files in link to this Project. (On the phone or Tab this would be a Camera button so just click away & it will upload here automatically)

Purpose:

Background:

Tracking Safety Incident in Contractor Foreman is critical to the continued success of your company. Contractor Foreman gives you the tools to track Incidents and create and track the Federally Required OSHA 300 Log. Without the proper tracking of Incidents and Safety related items, you can be exposed to large OSHA fines and higher insurance costs. Incidents is one of the tools Contractor Foreman provides you with.

Points to Remember:

Required fields are marked with an Asterisk (*.)

To save your work, click the blue “Submit” button.

To create a new employee write up, go to main menu and select “Employee Write Up.” Then select either the small “New” button to the right of “Employee Write Up” or select “Create a New Employee Write Up” from the top of middle pane (List View.)

Employee Write Ups

Employee: Select the Employee from the drop-down box.

Date: Enter the Date of the Write Up.

Discipline Type: Select the type of Discipline from the drop-down box. Your choices are; Verbal, Written, Indirect, Time Off and Termination.

Signature: If a signature was captured on the Phone App it will show here.

Write Up Images: Click the blue button “Select Images/Files to Upload.” This could be pictures of damage caused, files of previous infractions or any other type of documentation that is required.

Occurrence: Select the appropriate times violation occurred. Choices are First and Repeated.

Witnesses: Use the blue button to select any employees that were witness to the infraction.

Note: Enter any other information not in the other fields that may be important to this Employee Write Up.

Generated By: System will auto populate this field based on login.

View PDF and Email PDF Buttons: To view and save or print the PDF click “View PDF” to Email the Employee Write Up click “Email PDF.

Incidents

Incidents is one of the tools Contractor Foreman provides you with.

To create a new Incident, go to main menu and click on “Incidents” then click on either the small “New” button to the right of Incidents or click on larger blue button “Create New Incident” in middle pane. Required fields are marked with and Asterisk (*.)

Project: Select the appropriate project from the drop down. It also has defaults for Office, Warehouse, Driving and other as Incidents can happen anywhere.

Incident Date and Time: Fill out the date and time the Incident happened.

Type: Select the type of incident that occurred. Accident, Near Miss or Death.

Incident Description: Type a detailed description of what happened. You can add steps taken to correct later.

People Involved: Use the blue button to select who was involved. This is NOT where you put witnesses.

People Notified: Use the blue button to select who was notified of the incident.

When Notified: Use the blue button to show when people (usually management) were notified.

Witnesses:  Use the blue button to select who witnessed the incident.

Incident Images: Select and photos taken of the incident.

Incident #:  System will auto generate and auto populate this field.

Any Injuries: Most times an incident will not result in injuries. It is still important to document the incident. Select Yes or No box. If you select “Yes” a text field will open below where you need to fill out a detailed description of the injury.

Steps Taken to Correct: Write a detailed description of the steps that were taken to correct why the incident happened.

OSHA Violations?: Select “Yes” or “No.” If you select yes a text box titled “Describe OSHA Violation” will open.  Use this to describe the violation.

Note: This is a place to enter any other general notes.

IMPORTANT: If your company is required to complete OSHA 300 logs, Contractor Foreman makes it easy to add Employees to an incident.  Each Employee will have their own OSHA 300 line-item per OSHA requirements. Follow these steps below:

Click “Add Employee” button. A pop up will appear, fill out all fields.

Select Employee: Select Employee from drop down box.

Title: Add the injured employees title.

Where the event Occurred: Write a description of where the injury took place.

Classification: Select the appropriate item from the drop down. Death, Days away from Work, Job transfer or Restriction, or Other Recordable Case.

Days Away from Work: When known enter the amount of days the employee was off work.

Days on Job Transfer or Restriction: When known enter the amount of days the employee was on transfer or restriction.

Injury Illness or Type: Select from one of the items in the drop down.

Select Save to save the record. This will then take you back to the Main Incident Screen.

To save record click on the “Submit” Button

To Edit the record, click on the green pencil icon in the upper right of screen.
